Certification is not automatic once you graduated and having a diploma. There are requirements you need to settle before you can have your certificate as a registered medical assistant. There are associations that provide certification award diploma for medical assistants and it depends upon the association on how it will be process

Certification is more than a title, it only designate that a registered medical assistant was able to meet the standards of practicing the profession and therefore given a chance to be committed in their choice of work.
